Hatboro Pediatrics, PC has been recognized as a Level 2 Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). This recognition established Hatboro Pediatrics, PC as a model of care emphasizing care coordination and communication in an effort to transform primary care into a positive and beneficial experience for all of our patients.

A patient centered medical home chooses patient practices that promote partnerships between patients, doctors and medical teams versus treating a patient as just a series of impersonal office visits. Patients are treated by a pediatrician-led team that coordinates how their healthcare is delivered across the entire system, from physicians to behavioral health specialists, to nutritionists and other specialists. Hatboro Pediatrics, PC medical home demonstrates the benchmarks of patient-centered care with added features such as open scheduling, expanded hours and proven health information systems.

Hatboro Pediatrics, PC has implemented many changes and improvements to achieve this level of success. We could not have achieved this recognition without the efforts of our dedicated staff. As we move forward on our PCMH journey, the goal is to reach Level 3 recognition.

PCMH standards are aligned with the joint principles of the Patient-Centered Medical Home established with the American Academy of Pediatrics.
